• Application fee of 75 US$ sent to Secretary-Treasurer

Phase II credentials review:

• On-line credentials review application completed and submitted prior to October 1st, 2011
• Course transcript(s) sent to Secretary-Treasurer
• Copies or reprints of two publications sent to Secretary-Treasurer
• Application fee of 250 US$ sent to Secretary-Treasurer

Intent to take Phase I

• On-line notification of intent completed and submitted prior to February 1st, 2011
• Examination fee of $ 200 US sent to Secretary-Treasurer

Intent to take Phase II

• On-line notification of intent completed and submitted prior to February 1st, 2011
• Examination fee of $ 300 US sent to Secretary-Treasurer
